Screening questionnaire

This is the second and final step of joining the panel. Your enrollment request is sent when you submit this page. These questions help us match you to studies you qualify for. There are no right or wrong answers, and answering honestly is the fastest way to get invited. Your responses are treated as confidential under our privacy policy.

Question 1

This is where your study payments will be sent. Please type it carefully. Mistyped usernames can send a payment to the wrong account, and those payments are hard to recover, so we ask you to enter it twice and we'll also confirm it with you before your first payment. We'll never ask for your Venmo or PayPal password.

Question 8 Have you or a loved one ever filed a personal injury or medical malpractice claim?
Question 9 Have you ever served on a jury?
Question 10 Have you or a close family member suffered a serious injury?
Question 11 How would you describe yourself?
Question 12 Rate your agreement with the following statements.

1 = strongly disagree · 5 = strongly agree

Identity verification

We verify identity at enrollment. A driver's license, state ID, or passport works. Your name, photo, and date of birth are all we need, so you may cover the ID number before photographing it. ID photos are used only to verify your identity, are never shared with clients, and are deleted once you're verified. At session check-in we'll briefly confirm on camera that you match this ID.

Your ID photo uploads securely with this form when you submit. Files up to 8 MB, image or PDF.

Agreement Review and accept

Submitting sends your enrollment request, questionnaire answers, and ID photo to our recruiting team. You'll see a confirmation page when it's done.

Some studies must exclude people with certain jobs, affiliations, or connections to the subject matter, so your answers determine which studies you qualify for, not whether we value your participation. Providing false answers to qualify for a study is grounds for removal from the panel under our terms of service.
